A composite, not a real company. The pattern turns up most years, the people were invented, and no business is named anywhere on this page.
Say a distribution business with about twenty people is renewing a facility. The bank asks for the last two years and an explanation of a dip in one quarter, and they would like it by Friday.
The bookkeeping is done by a firm that bills monthly and closes about six weeks behind. The returns are prepared by somebody else who last spoke to the owner in the spring.
The owner forwards the request to the bookkeeper, who sends a report. The bank reads it and asks why one figure moves. The bookkeeper explains that an adjustment was made at year end by the preparer, and suggests asking them.
The preparer replies on Thursday, working from a file that no longer matches the one the bank is holding. By Friday the owner is the only person who has seen both versions, and the owner is the one who can explain neither.
Nothing anybody did was careless. The record was accurate as far as it went, and the adjustment was correct. The two versions simply never met, because nobody owned the space between them.
The dip in that quarter had a perfectly good explanation as well. It took four days to surface because the person who knew it was never the person holding the file.
The request arrives, the months are already closed, and the year end adjustments are already in the record because the same desk made them. The explanation of the dip gets written that afternoon, by the person who watched it happen.
The bank gets one file and one voice. If they come back asking for something signed, the referral happens on Tuesday instead of the following month.
A file the bank accepted, an explanation nobody had to reconstruct, and a clear view of which row would ever have needed somebody else.
Nobody real appears anywhere in this. The people were made up to carry a pattern, and the pattern is the only part taken from life.
Neither job title is a CPA credential, and this desk does not hold one. Audits, reviews and assurance opinions stay with a firm that holds the credential, even when one person keeps both the record and the decision. If a lender wants a signed report, the meeting ends on a referral rather than a workaround. The whole of it is written out on the disclosures page.

Breadth is the lender row on the strip having exactly one owner, so the file and the explanation come out of the same place in the same week.
Height is the October row, where a dip like that would have been talked about while the year could still absorb it, instead of defended afterwards.
I thought I needed a better bookkeeper. What I actually wanted was one person who could answer for the whole file.
You will be talking to the Steven Palmieri practice. A sentence about the Tuesday split is enough to start.
